About this service

Virtual Care at HealthSmart Medical

Convenient phone or video visits for appropriate medical concerns and follow-ups.

Virtual care allows patients to speak with a clinician by phone or video when the concern can be assessed safely without an immediate in-person exam. It can be helpful for follow-up, medication questions, result review, chronic disease check-ins, some minor symptoms, mental health discussions, and certain forms or referrals. At HealthSmart Medical in Scarborough, virtual care is used when it is appropriate for the concern and when the patient can communicate clearly from a private setting.

Not every concern is suitable for virtual care. Some symptoms require an in-person exam, vital signs, swabs, urine testing, blood pressure measurement, ECG, procedure, or urgent assessment. Severe chest pain, stroke symptoms, severe shortness of breath, fainting, major injury, severe abdominal pain, heavy bleeding, confusion, or rapidly worsening illness should not wait for a virtual appointment. Emergency symptoms require emergency care.

Virtual care works best when patients prepare. Have a health card, medication list, pharmacy name and fax number, allergies, recent test results, home blood pressure readings, blood sugar logs, temperature readings, symptom timeline, and questions ready. If the appointment is by video, choose a quiet, well-lit space. If by phone, keep the line available and answer calls from unfamiliar numbers around the appointment time.

Medication reviews are a common use of virtual care. The clinician may discuss whether a medication is working, whether side effects are present, whether refills are appropriate, and whether monitoring is due. Some medications require blood work, blood pressure checks, in-person assessment, or follow-up before renewal. Patients should not wait until the last dose if they know a refill visit is needed.

Test results can often be reviewed virtually. The clinician can explain what results mean, whether anything is abnormal, whether repeat testing is needed, and how results connect to symptoms. Patients should remember that normal results do not always end the assessment if symptoms continue. Abnormal results may require in-person exam, medication changes, imaging, referral, or urgent care depending on severity.

Mental health concerns may be suitable for virtual care when the patient is in a safe, private place. Anxiety, depression, stress, sleep problems, grief, burnout, and medication follow-up can often begin with a phone or video discussion. If there are safety concerns, suicidal thoughts, psychosis, severe withdrawal, or immediate risk, urgent or emergency support may be needed instead.

Some minor symptoms can be discussed virtually, but limitations matter. A rash may need photos or in-person viewing. Ear pain, abdominal pain, chest symptoms, shortness of breath, injuries, urinary symptoms, and infections may require examination or testing. The clinician may start virtually and then recommend in-person care if the information is not enough to make a safe plan.

Prescriptions, requisitions, and referrals may be arranged through virtual care when medically appropriate. A virtual visit does not guarantee a prescription, antibiotic, note, or referral. The clinician still needs to assess the concern and decide what is safe. If a pharmacy is needed, patients should provide the correct pharmacy location and contact information.

Privacy and communication are important. Patients should avoid virtual appointments while driving, working in a noisy place, or sitting somewhere they cannot speak openly. If an interpreter, caregiver, or family member is present, the patient should identify them. The clinician may need to confirm identity and location, especially if urgent advice is required.

Virtual care also depends on reliable follow-through. If the clinician orders blood work, imaging, swabs, or an in-person follow-up, the patient should complete those steps and book review as advised. A virtual appointment can start a plan, but it may not finish the assessment. Patients should also make sure the clinic has their current phone number and preferred pharmacy so instructions, prescriptions, or follow-up needs are not delayed.

Patients with home monitoring tools should have recent readings available when relevant. Blood pressure, pulse, temperature, oxygen saturation, blood sugar, weight, peak flow, or symptom diaries can make virtual care more useful. These readings do not replace clinical assessment, but they can help the clinician decide whether virtual management is reasonable or whether in-person care is needed.

At HealthSmart Medical, virtual care is meant to improve access while still respecting clinical safety. The visit can answer questions, review results, adjust plans, arrange follow-up, and decide whether in-person care is needed. Patients should leave knowing the plan, what warning signs matter, and what to do if symptoms change.
